create headings for top-level chapters "operational" and "development"

we have different folders for operational and development now (#116 (closed)), but this should also be reflected in the chapter hierarchy (#115). so new top level headings need to be created, and all sub chapters need to be moved one heading level down (is there a way to automate this?)
