Clean up: consistently use spaces instead of tabs (Matlab standard), remove author and last-modified-notes, formatting fixes

Moved construction of CTF-regularization-matrix to separate private-function to avoid code repitition

More consistent order of input arguments in FresnelPropagator (+ changed other functions to match new signature of the constructor)

Modified getPhantom to directly return the phantom instead of struct and adjusted examples in other files accordingly
